Recognizing that planning is required in order to provide continuity in the student’s educational program and to arrange the appropriate staff and resources, a parent/guardian of a student in Grades Pre-K through 8 must use this form to request a transition from in-person or hybrid services to full-time remote learning or vice versa. Please note: Westfield High School families should use the form found under "Family Resources" on the WHS webpage.
A parent/guardian of a student who wishes to transition from hybrid, in-person instruction to full-time remote learning must submit a request using this online form at least two days prior to the requested start date of full-time remote learning so that teachers can be notified.
To allow for optimal continuity of instruction and the time needed for administrators and teachers to make the arrangements to effectively serve the student, a request to transition from all-remote learning to hybrid, in-person instruction must be submitted via this online form at least two weeks before the start date of hybrid, in-person learning.
IMPORTANT: If you are requesting to transfer your child to remote learning due to possible COVID-19 symptoms or exposure, you must contact your school health office for further guidance.
