Recognizing that planning is required in order to provide continuity in the student’s educational program and to arrange the appropriate staff and resources, a parent/guardian of a student in Grades Pre-K through 8 must use this form to request a transition from in-person or hybrid services to full-time remote learning or vice versa. Please note: Westfield High School families should use the form found under "Family Resources" on the WHS webpage.
As we work to bring our students back for more in-person instruction safely, we ask every parent with students in Grades K-8 to commit to either hybrid or all-remote learning through April 19. The deadline has passed for students in Grades 6-8 to make a change.
Attention parents with students in Grades K-5: If you wish to make a switch, please complete this form no later than Monday, March 8. This will be your choice through April 19. No action is necessary if you do not want to make a change. Families will have one more opportunity to make a change request in early April regarding their choice for the last quarter of the school year.
It remains all about the guidance from the CDC and the number of students we can safely accommodate in our classrooms. The district will use this data to help inform our decisions.
IMPORTANT: If you are requesting to transfer your child to remote learning due to possible COVID-19 symptoms or exposure, you must contact your school health office for further guidance.
